EVOLVE Family Services Photo and Video Release
I, hereby grant permission to EVOLVE Adoption & Family Services, to use my image, in photos, video or still form, as well as my voice as it is captured on audio, video. I acknowledge that my likeness may be used in finished works that are altered, copied, shown, published, or distributed.
· I authorize EVOLVE Adoption & Family Services to distribute and reproduce the materials for the following purposes: Advertising, marketing, branding, educational, digital promotions, videos, online courses, media, other commercial or non-commercial purposes.
· This authorization extends to all languages, media, formats, and markets now known or later discovered.
· This authorization must remain in effect unless I cancel it in writing.
· I renounce my right to review and approve any finished product bearing my likeness.
· I acknowledge that I consented to this release voluntarily and without payment.
· I understand that all photos and videos that will be taken in this activity are copyrighted by EVOLVE Adoption & Family Services.
· I thus absolve and discharge EVOLVE Adoption & Family Services from any claims, demands, and causes of action that I, my heirs, agents, administrators, executors, or other representatives may assert on my or my estate’s behalf.
· Both the duration of this release's validity and the areas in which these items may be distributed are unrestricted.
